Welcome to the Prehistoric Museum at Utah State University Eastern
Due to the current guidelines associated with the Governor's Utah Leads Together Covid-19 plan, all visitors to the Prehistoric Museum must pre-register with this form before coming to visit.

Prior to visiting, Visitors will

·      Be required to share their contact information.
·      Check a box that they are not symptomatic so that we can trace visitors should an outbreak occur.
·      Check a box that they have reviewed visitation requirements and will comply (wearing a mask if possible,
       maintaining social distance with those outside of their group, and wash hands upon entry).
·     The giftshop is limited to one group per side at a time and merchandise will be handled by clerks until
       purchased
·      We ask that you pay with credit or debit cards
·      Follow all directional markers and social distancing guidelines
·       Utilize the elevator only if necessary due to mobility or health issues
