Please complete this short form to confirm the item(s) you intend to donate to the 2023 Sweets for Success fundraiser. We appreciate your timely response as we work to finalize plans for this crucial fundraising event :) The deadline is March 29th. Please note that dessert descriptions received after the deadline may not be included in the auction program.

The event is Friday, April 14, 2023. Instructions for dessert delivery are below and will be emailed to you the week of the event, you may also request pick-up within this form. Desserts will be auctioned off during the event and all proceeds benefit Main Street West Branch (501c3 nonprofit).

All bakers are welcome! You do not need to be a professional. :)

Sweets for Success will be held on Friday, April 14th at Little Lights on the Lane in West Branch, IA.

I will drop off my donation in West Branch on Thurs, April 13th (9-5 at Rummells Center or 5-7pm at Little Lights).

I will drop off my donation to Little Lights on Fri, April 14th between 10AM-NOON.

My donation will need to be picked up (Thursday, April 13).
