I agree to my child taking part in the SHIFEN TRAIL 十分越野 (EVENT) and confirm and acknowledge all of the following:

本人同意我的小孩參予本次 SHIFEN TRAIL 十分越野 (EVENT)，並且我已詳讀以下事項，並已同意與遵循:

1. I confirm my child is to the best of my knowledge in normal good health and I will make the organizers aware of any pre-existing condition for which my child may require routine or self administered emergency medication. e.g. Asthma

1. 我清楚並至知道我的小孩身體正常且健康狀態一切正常與良好。我將主動告知主辦單位，我的小孩有常用藥物或正服用相關藥物，並且自行備好相關藥物做為緊急使用。如氣喘等...

2. I confirm that my child is in sufficient physical condition to safely participate in the activities of the EVENT and that he/she will take full responsibility for his/own own safety during the EVENT.

2. 我清楚並確信我的孩子有足夠的身體與心靈條件安全的完成本次活動，並且她將會對自己在活動中發生的事情負全權責任。

3. I confirm that my child is capable of performing all activities associated with the EVENT.

3.我清楚並確信我的孩子能有能力參予此活動與此活動相關的活動。

4. I understand that whilst the organizers, the activities personnel, and other persons involved with the EVENT will take all reasonable care, there remains an element of risk in participating in physical activities.

4. 我清楚與明白，即使主辦單位與相關協辦人員將會謹慎對待此次活動相關的事宜。但參與體育活動仍有一定的風險存在

5. I therefore accept the organizers, event personnel, sponsors and all persons involved with the EVENT will not accept responsibility for any loss, damage or injury suffered by persons and/or their property arising out of or during the EVENT, unless such injury, loss or damage was caused by, or resulted from, negligence or deliberate act.

5. 因此，我接受主辦單位、活動人員、贊助商及所有與活動有關的人員不承擔任何因活動或活動過程中發生的人員及/或其財產的損失、損害或傷害，除非該等損失、損害或傷害是由疏忽或故意行為造成或導致的。

6. I accept Photographs and Videos may be taken for the purposes of recording the event and future promotion and advertising.

7. In an emergency, I authorize the organizers to carry out first aid and/or hand over my child to trained medical personnel for treatment.

8. I have made my child aware of the risks associated with the physical activities of the EVENT.

8. 我已經讓我的孩子清楚意識與明白到與體育活動相關的風險。

9. I have made my child aware of the rules associated with the EVENT.

9. 我已經讓我的孩子清楚意識與明白到與事件相關的規則。

10. My child can ONLY participate in the category registered for the EVENT.

11. I am the participants' parent or guardian.
