Use this form to request support for SOS including requesting a meeting, trainings, portal licenses, materials, and other needs from the TCMHS Prevention Team.
Trainings:
- You can request an implementer training, Trusted Adult staff training and Trusted Adult parent training. Only the person/people in charge of planning SOS implementation must complete the implementer training, provided by the Youth Suicide Prevention Specialist (YSPS). All school staff must complete the Trusted Adult Staff training. The Trusted Adult Parent training must be made available to all parents before implementing SOS.
- SOS suggests that the school implementer facilitates the Trusted Adult Staff and Parent trainings. However, if you would like to have the YSPS facilitate these trainings instead, you can make these requests below.
Portals:
- The SOS portal is where you can find all of the materials you need to implement SOS. The license for the portal lasts for one year and must be renewed every year.
- If you need help with funding or are unsure of how to renew your portal, complete this form.
Materials:
- Request materials such as posters, wallet cards, stickers, and more!
