You MUST fill out this form and successfully submit it to be allowed on the ice or bounce houses AT ANY TIME

I know that participating in activities related to working at a Wenatchee Wild hockey game is potentially hazardous.  I assume all risks associated with participating as an intern or volunteer during the 2023-2024 season including, but not limited to falls, hit by a puck, contact with other participants, effects of the weather, and the condition of the working environment, all facts being known and appreciated by me.  Knowing these facts, and in consideration of you accepting my usage, I hereby myself, my heirs, executors, administrators or anyone else who might bring claims on my behalf, covenant not to sue, and waive, release and discharge the Wenatchee Wild Hockey and/or Shoot The Puck Foundation, and/or the Western Hockey League, U.S.A. Hockey, the Town Toyota Center and its management, all charities benefiting from this event, volunteers, committee members, and any and all sponsors, from any and all claims of liability for death, personal injury, or property damage of any kind or nature whatsoever arising out of or in the course of participating in this event.  This Release and Waiver extends to all claims of every kind or nature whatsoever, foreseen or unforeseen, known or unknown.  Be it known, the Participant assumes any, and all, responsibility for any maintenance or repairs that must be made upon returning the equipment (such as the Wild’s hockey gear).  This is relevant in the case where equipment is damaged or broken.

I UNDERSTAND THAT THIS IS A LEGAL DOCUMENT.  I HAVE READ AND UNDERSTOOD THIS RELEASE, AND I UNDERSTAND ALL ITS TERMS.  I EXECUTE IT VOLUNTARILY AND WITH FULL KNOWLEDGE OF ITS MEANING AND SIGNIFICANCE.
