> All new and returning athletes MUST complete this form if they want to participate in SNOWBOARDING.

> Completing this form will help us prepare for the season and provide contact information to the Head

Coaches.

> If your local program does not offer SNOWBOARDING you may sign-up and participate with a nearby

county. Keep in mind you will have to provide your own transportation.

> Signing up to participate in this sport does not guarantee participation as participation is dependent upon an

adequate number of volunteers to ensure we provide safe and meaningful sports training.
