I, (type name below), agree and understand that I am adopting a rescued cat. After adoption, I understand that this is now my cat and my responsibility from here on out. Purring Hearts VA is not responsible for pre-existing conditions or any incurred medical bills from this moment on, and all known medical conditions have been disclosed. Purring Hearts VA always recommends that you have your new cat seen by your vet within the next 30 days. Purring Hearts VA works very hard at saving every one of these cats from death. Many are from absolutely horrible situations or have backgrounds we don't know. You are their second chance at life. Purring Hearts VA does the core vaccines, FIV/Felv test, Spay/Neuter, and deworming. We have gotten them to a place that it is safe to pass the baton, and you, as their new family, can continue to be their savior. Thank you for being part of our rescue family.

Please carefully read each item below, and check each statement to show that you have read and agree to each one. *

Any breach of this contract could result in PurringHeartsVA taking steps to recover the adopted animal. PurringHeartsVA acts solely in the best interest of the animal.

The animal may not be sold, given to, or adopted to anyone else unless agreed to by PurringHeartsVA.. There shall not be any transfers of ownership of the adopted animal to anyone else, or to a firm, or to a corporation, or to another organization for homeless animals. The adopted animal must be returned to PurringHeartsVA if for any reason you are no longer able to care for the animal. Upon the safe return of the animal, in good health, and with all current medical records, this contract will no longer be valid.

Any violation of this agreement will result in court costs and attorney fees payable by the adopter if necessary for PurringHeartsVA to redeem the animal.

I understand that PurringHeartsVA cannot guarantee the health, behavior, age or temperament of the animal. The adopter understands that even though vet checks and vaccinations have been performed, future illnesses may arise. The adopter agrees to provide the animal with vet care in such an event.

In an effort to help resolve issues early, the adopter is encouraged to notify PurringHeartsVA of any behavioral issues with the adopted animal.

I understand that the care of this animal requires a longterm commitment of caring love and responsibility for the lifetime of the adopted animal. The cat/kitten will be kept INDOORs only as a family pet and the animal will not go outside for its lifetime unless secured by leash with body harness or secured within a locked carrier/stroller.

The adopter understands that the animal shall not be used for any kind of research. The animal may not be used for any illegal purposes. The animal may not be used for food or eaten.

PurringHeartsVA reserves the right to perform home visits to check the living conditions of any adopted animal. The adopter agrees to allow such post-adoption home visits. If PurringHeartsVA deems that the adopted animal is not being treated properly, or is living in unacceptable conditions, PurringHeartsVA reserves the right to take ownership of the animal immediately.

The adopter agrees not to abuse or neglect the animal in any way. The adopter affirms that no member of the household has been convicted of an animal welfare law violation such as neglect, cruelty, abandonment, etc.

The adopter will notify PurringHeartsVA if there is a change in contact information. (Home address, phone number or email address)

The adopter will notify PurringHeartsVA in the event of an untimely death of the animal.

In the event that the animal gets lost or escapes from the adopter, the family shall immediately contact PurringHeartsVA and take the steps to recover the animal including posting fliers, placing lost animal ads, calling and physically going to local animal shelters to locate the animal. As well as calling veterinarian offices and asking them to keep a lookout for the animal.

The adopter will NOT have the cat/kitten declawed.

In the event that I am unable to make it to Petsmart Shortpump to complete the Ipad Charity Tracker, I give Purring Hearts Va permission to fill out my Name, Address, Phone Number and Email.

PurringHeartsVA deems these things as proper care and treatment of the adopted animal. The adopter is to check each item as they understand it to be proper care and treatment of the adopted animal. This section applies to all adopted animals: *

Fresh water to be provided at all times. Cat's prefer running water which helps with urinary care.

The animal will be fed wet food twice a day (AM and PM) in the amount that is appropriate for the size, weight and age of the animal. Dry food will be left out all day and night.

All yearly vaccinations will be kept up to date with vet or low cost animal clinic.

Once a year veterinary checks will be done with personal vet of choice or low cost animal clinic.

The adopter will get prompt medical care for the adopted animal in the event of illness or injury.

Monthly flea treatments will be provided, if other animals that reside in the home, go outside.

The animal will be kept inside only, as part of the family. (Unless otherwise discussed, i.e.: barn cats)

The adopter will provide mental and physical stimulation for the adopted animal according to the animal's needs. Climbing towers and scratching posts and toys help.

The animal will be kept clean and groomed as it is required for the animal's needs. Cats are self-bathing and do NOT require regular baths from the owner.

The adopter will do his or her best to be informed of pet food and treat recalls.

The adopter understands that it may take up to 3-4 weeks (or longer) for the adopted animal to adjust to his/her new family life. Starting the animal in a small space, without other family pets, is essential for them to adapt well.

A snug fitting break-away collar is only recommended if the animal is deemed a flight risk from within the home. Animals are more likely to be rescued from outside if wearing a collar.

The adopter will register the animal's microchip within one week of bringing the animal home. This service is free.
