Smithfield Elementary School: Transfer Request between Full Remote Academy and In-Person Rotation

Families who are experiencing a significant hardship may request a transfer between the district’s current attendance plan and the Full Remote Academy prior to the end of the semester. To be eligible for a plan change, families must meet at least one of the criteria listed below.

Request Process:

1. Complete this form in its entirety.
2. All requests will be reviewed by Allison Plunkett and Jill Trotter.
3. Marlene Henderson, Registrar, or Elizabeth Alibudbud, Financial Secretary, will follow up within 5 business days.
