Dear Parents,
Due to the COVID-19 pandemic, we are concerned for the well-being of all students.  In order to promote as safe an environment as possible, we are screening all students.  Please answer each of the questions below with complete truthfulness, provide your identifying information for each child who attends a Union City public school.  Any student who is sick, running a fever, or who has travelled outside of the immediate region should contact the school for guidance before returning to the building.

Debido a la pandemia de COVID-19, nos preocupa el bienestar de todos los estudiantes.  Para poder tener el más seguro ambiente posible, estamos evaluando a todos los estudiantes.  Favor de responder a todas las siguientes preguntas sinceramente, proporcionar la información de identificación para cada niño que asiste a una escuela pública de Union City.  Cualquier estudiante que esté enfermo, tenga fiebre o que haya viajado fuera de la región inmediata debe comunicarse con la escuela para recibir dirección antes de regresar al edificio.
