About this survey
We’re HMRC researchers looking at HMRC's forms and ways of making them better.

One group of forms, the print and post forms, must be completed online, printed and posted to the user's employer or HMRC, sometimes with supporting materials.  It is these forms that we are interested in hearing your thoughts on for example the 64-8, CWF1 or CIS305.

For this survey, we are looking to understand more about your interaction with the print and post forms so that we can understand if there are opportunities to improve the user experience.

The information you provide will be used to improve HMRC services. It will be fully anonymous and only shared internally. It will be stored securely and fully in compliance with GDPR. If you choose to give us your email address for further contact, this will be stored securely, encrypted and password-protected. Only the researchers on this team will have access your data.

The survey should only take 10 minutes to complete and is entirely optional.
