Description of in person programs offered
IN PERSON Mondays March 1, 8, 15, 22   Arts and Crafts 4:00 - 6:00

Youth will come together in person Mondays in March and participate in Art and Craft activities.

Youth will come together on Tuesdays in March and participate in Science activities.

IN PERSON Wednesdays March 3, 10, 17, 24 Computer and Video Game Lab 4:00 - 6:00
Youth will come together on Wednesdays in  March to participate in Video games, computer and other electronic devices.

Snack will be served at 4:00  when youth arrive. Does the youth listed above have any food allergies or special dietary concerns? (Including Vegan/vegetarian, gluten intolerance, etc.) Please list below.
If you would like to give your youth permission to leave MYC on their own at anytime during programming (4:00 - 6:00) indicate below. (Other wise all youth will be released to leave at 6:00 pm)

I give permission for my child to be photographed  during the time he/she is involved in activities at the MYC. Photos of youth participating in activities may be used on our website or facebook page.

Please read the following statement of COVID19 safety policies and procedures and type your name below to sign & confirm you and your youth's understanding.  
•Both staff and youth will be expected to maintain a 6 foot distance with other members.

•Both staff and youth will be expected to wear masks during their time at MYC.

•Youth and staff will have their temperature taken and answer health screening questions at arrival each day, if their temperature is above 100F they will be asked to leave until tested for COVID19.

•Parent's and youth agree that youth will not come to MYC on days they are feeling ill.

•Youth members will be expected to follow COVID19 safety policies and procedures. If a youth is unable to do so a parent will be contacted and youth will be asked to take a break from MYC activities.

•Parent's will contact MYC immediately if a youth member tests positive for COVID-19  (MYC will NOT share names or personal information regarding a positive COVID19 result, but will implement contact tracing procedures).

•If a youth does not currently have a pledge form on file with MYC, a staff member will reach out with the paperwork.
