本報名表含5個區段，請同學確實填寫。依招募公告，需另將個人申請資料電子檔上傳至NTU Space雲端並於報名時提供下載連結，方可完成報名程序。本處將依報名時提供連結（請設為公開）下載資料，報名完成後即不可更新檔案。

招募報名時間自11月1日(二)至 2023年1月13日(五)中午12:00止。
