AMS 2024 Tutoring Request Form  

Tutoring will be held at AMS. It is FREE! 

Students will:

- be consistent with attending tutoring. 

- be able to bring their course work to receive support. 

- sign in for each tutoring session. 

*** Please note - Students may choose 2 sessions (AM and/or PM) on a first come, first served basis. They are subject to wait list, by grade level, and by subject. If the session(s) chosen are at full capacity, they will be scheduled for the next available session. 

Parents will:

- be able to sign up through a Google Form monthly. 

- be expected to pick up students five minutes prior to pickup time. 

*** Please note - Three (3) consecutive unexcused absences, tardies, and/or late pickups will result in the student being removed from the tutoring program.
