Commencement
• The swimming program is exclusive to the Singapore Swimming Club members and their registered guests. All guests must also be registered in order to be accepted into the program.
• The Adult Learn to Swim Program is open to adults aged 18 and above only.
• There will be a registration fee of $21.60 (inclusive of GST)
• Availability of selected sessions will be on a first-come, first-served basis.
• Please DO NOT commence lessons until confirmation by either Letter of Commencement or email confirmation from the Swimming Department.
• All payments will be debited to the member account accordingly.

Termination
• Termination of lessons verbally will not be accepted. Members who wish to terminate lessons must give the notice in writing to the Swimming Department by Email.
• One clear month's notice in writing to the swimming department is required for withdrawal from the program.
• Letters received dated 1st to 20th will take effect on 1st of next month.
• Letters received dated 21st to month end will take effect on 1st of the next following month
• Request for IMMEDIATE TERMINATION of lessons must be supported by official medical certifications.
• Upon receiving the termination letter, an acknowledgment will be sent to confirm the effective termination date. Please call the Swimming Department if you do not receive the acknowledgement within 2 weeks.
• Temporary cessation is not allowed. Members are requested to give the required notice to terminate lessons and re-submit a new registration form to rejoin lessons.
• Should a swimmer wish to rejoin the program within 9 months of the withdrawal date, a reinstatement fee of $108 (inclusive of GST) shall apply.

Important
• All matters pertaining to all swimming lessons must be referred to the Swimming Department. Verbal confirmation between members and coaches on all matters concerning swimming lessons will not be recognised.
• Swimming Lessons for Learn-to-Swim Adult Swimming Program will not be conducted on Public Holidays.
• There is no replacement of classes for any reason.

Personal Data Protection Act – PDPA
• In line with the government’s policy on promoting a balance between the need to protect your personal data and the swimming department’s need to obtain and process such data for communication, we seek your clear and unambiguous consent to receive notices, events updates, events forms, swimming program and events related articles, swimming classes related matters, swimming program and event-related finance matters, and collaterals from Singapore Swimming Club.

• Under the Personal Data Protection Act, we will not be able to send you any notices, events updates, events forms, swimming program and events related articles, swimming classes’ related matters, swimming program and event-related finance matters, and collaterals from Singapore Swimming Club. Photographs of the swimmers may be used for publicity purposes in the club magazine. By submitting this form, I understand and give Singapore Swimming Club permission to use the photographs, contact number and email for the above-mentioned purpose.

Indemnity
• I am fully aware of the risk involved and I shall not hold Singapore Swimming Club, its organizing committee, or any member of staff responsible for the death, injury, or loss arising from any cause whatsoever during the lesson. I also agree to comply with terms and conditions pertaining to this training program.
