The Board of the Kalamazoo Junior Symphony Orchestras (KJSOs) seeks for any in-person activity of the KJSOs during the COVID-19 pandemic to be conducted in a safe manner. For such activities, mitigation protocols will be followed, including, but not limited to, masking, distancing, instrument PPE, and shortened contact hours. However, no absolute assurance can be provided as neither the Board members nor the KJSO staff are medical experts or have specialized training. To that end, we ask that all members participating in in-person activity, as well as their parents/guardians, agree to the following conditions:

1. Certification of Medical Condition. Prior to a member’s participation on any given date, he or she will not have:
a. Experienced any symptoms associated with COVID-19 for a period of 14 days prior to the member’s date of participation;
b. Been exposed to a confirmed or suspected case of COVID-19 or have been diagnosed with COVID19 and not yet cleared as non-contagious by public health authorities or the health care team responsible for his or her treatment.

2. Authority of KJSOs Staff. KJSOs Staff shall have full discretion to add any terms and conditions that they may deem necessary for in-person activity to ensure the health and safety of all participants, including the exclusion or dismissal of any member for failing to meet such terms and conditions.

3. Acknowledgement of Risks. All members and their parents/guardians acknowledge and understand that participation includes possible exposure to COVID-19, and assume the risk of injury, harm, or loss associated with such in-person activity.

4. Agree to Disclose Positive COVID-19 Test. All members and their parents/guardians agree to disclose to others involved in such in-person activity a positive COVID-19 test within 14 days after participating in such activity.
