Urban Impact, Inc. is conducting this survey of district businesses due to COVID-19. It takes about 10-15 minutes to complete and will be open until September 29. It has been extended into October. It has once again been extended through Dec. 4, 2020, as a final deadline. As a way to help out and as an incentive for the completion of the survey, businesses located in the 4th Ave. Business District will receive a $200 Visa Gift Card that will be administered early-to-mid October. Additionally, your answers will help us help your business more and drive development in the Fourth Ave. Business District and Birmingham Civil Rights District. This is a confidential survey.
