In order for your event to be added to the DDCA calendar, you must have submitted this form to the DDCA Board Chair AND have submitted payment to the DDCA Treasurer for your application fee.
Event Application Fee Received by DDCA:
Before May 31st = $200
Before October 31st = $250
After October 31st: Add $100 late fee
