CJHS & CHS Golf Athletic Acknowledgement of Risk & Consent Form

Please submit this form for each individual student.


This form must be completed prior to tryouts for any sport within the school or involvement with extracurricular school athletic activity.  By submitting this form, you acknowledge that your child may participate in Westwind School Division extracurricular athletic activities or teams (including tryouts).

Athletics are an important part of the Westwind School Division culture. Young people learn a great deal from participation in interscholastic athletics programs. Lessons in sportsmanship, teamwork, competition, and how to win and lose gracefully are integral parts of the athletic programs at our schools. Athletic competition also improves school spirit and helps students develop pride in their school.

Athletic activities involve certain risk, dangers and hazards to the participants. These may include, but are not limited to, personal injury, death, property damage, expense and other loss, delay or inconvenience, and trip or event cancellation or curtailment.

Date: September 17th, 2024

Start: 10:00 am Shot-gun Start

Who: Grade 7 and 8 students

Check-In Time: 9:30. Coaches will check-in for the team.

We will have a bus take students to the golf course and back to the school when they are done.

Entry Fee: $30 per participant. Please by by cash or e-transfer (cjhsfees@westwind.ab.ca). Cards can be used in the office but cash/e-transfer is preferrable.

Information:

Will play from yellow tees and play the front 9 holes. 

Pull Carts: Bring a pull cart if you wish. There are very limited (4) pull carts available at the golf course so if you really want one, bring one.

On the first tee players will:

Introduce themselves to their competitors and exchange scorecards. Each player will keep track of someone else's score. It is important to write down the correct score and write it legibly. Max of 10 strokes per hole. Once you’ve reached 10 strokes, pick up your ball and proceed to the next hole.

Be reminded of the importance of “Pace of Play” and their expectation to stay with the group ahead of them.

Announce the ball (with identifiable markings) that they are putting into play, to ensure that two players are not playing the exact same ball.

Be encouraged to hit a provisional ball if the first ball is in danger of being lost or out of bounds.

Once the round is completed:

The players will submit their signed scorecard to their coach who will confirm the score and check for legibility.
