The Community Health Representative (CHR) Summit VI "Building Healthier Communities through Integration, Best Practices, Advocacy and Culture" planning committee would like to recognize and honor the service of a Tribal CHR Team/Program to be nominated by their peers. The winner will be recognized at CHR Summit VI Awards Lunch on May 6, 2020.

CHRs are significant contributors to healthcare teams focused on providing comprehensive care. CHRs have been shown to improve patient outcomes, reduce costs for healthcare and assist community members in avoiding unnecessary hospitalization and other forms of more expensive care. Currently, CHRs are estimated to represent 30% of the total community health worker workforce in Arizona.
