This form must be completed by both the mentee and their parent or guardian. The required approval must be provided by the parent or legal guardian. If you are over the age of 18, please use the adult application. It can be found on our website. Or, our program staff can provide the appropriate link upon request. Eligibility Requirements are as follows:

• Must be aged 14-22. If you are not yet 14,mw we will keep your application on file until you reach that age.
• Must be participating in an educational program. This includes middle or high school, a post-secondary
        educational institution, or an adult rehabilitation program if your plan is to transition to a college or
        university following training. Unfortunately, we cannot accept individuals who are not pursuing an
        educational program.
• Must have an open case with the Texas Workforce Commission's Vocational Rehabilitation Services.
• Must be legally blind.
