Consent
I hereby give my approval for my child’s participation in any and all activities associated with the program named above. I understand that accidents and injuries may occur during or while traveling to or from the program activities, and I assume all associated risks.
In the case of an accident, I give permission for the EdGE Learning Centre and its agents to administer any first aid treatment deemed necessary for my child's wellbeing, including transportation in an ambulance or private vehicle to the hospital.
Release of Liability
I agree to release, hold harmless, and indemnify the Bulkley Valley Collaborative Learning Society, The EdGE Learning Centre, and the
ir directors, officers, members, employees, instructors, volunteers, representatives and participants (hereafter collectively referred to as “the Releasees”) from liability for any and all damages or injuries incurred as a result of participation in the above named program, whether or not caused by active or passive negligence on their part, and to waive any and all claims that I have or may have in the future against the Releasees.I agree that this contract shall be binding upon myself, my heirs, executors, administrators, and representatives.
I agree that this contract shall be interpreted solely in accordance with the laws of British Columbia and no other jurisdiction, and that any litigation involving the parties to this contract shall be within the exclusive jurisdiction of the province of British Columbia.
