2023 "Cornhole for a Cure!" Entry Form
Use this form to enter your two-person team into our 2023 tournament which will be held on Friday, May 19th. Please enter your team by Tuesday, May 16th.

Prizes will be given for tournament winners, runners-up,  "Best Team Name" and "Best Team Uniform"!

Teams will be randomly placed in a double elimination format.

Due to the nature of this recreational event, all players will be required to sign a waiver before participation.  Please indicate your acknowledgment here. *
