Guidelines for completion

1. Read the related documents on the institution of the “Ministry of Catechist”before responding to this questionnaire.,

2. For the purposes of this questionnaire, "Catechist" or “Instituted Catechist” refers to a "Catechist who participates in the Programme and whose ministry of Catechist is conferred by the Bishop".

3. Use "⩗" to indicate the overall degree of agreement with the statement:

          1 - Strongly disagree 

          2 - Disagree

          3 - Neutral/No comment

          4 - Agree

          5 - Strongly agree

4. In the space provided, write your comments with respect to the statement / description concerned.

5. Fill in the relevant sections of the questionnaire based on the stakeholder group you belong to (stakeholders include leaders of diocesan organisations, priests of the Diocese / religious orders, deacons, religious brothers / sisters, catechetical instructors / assistants for various target groups, persons in charge of various working groups / service units in parishes, the general church members).
