Day of Caring Sign-Up Form
Agencies may use this form to sign-up for DOC projects on Wednesday, May 24th, 2023. Please read the following information regarding the day of:
All projects should start no earlier than 8am, and last until no later than 5pm.
All participating volunteers and up to 4 agency representatives will be eligible to attend a thank you luncheon at the Hillcats Stadium from 11:30-1pm.
Information for agencies to sign-up for shirts (up to 4) and lunches (up to 4) will be emailed in a separate form prior to the event.
Please be sure to note ALL necessary equipment for your projects, and whether or not they will be provided by your agency or if they should be provided by the volunteers.
Please sign each project up individually- this form can be used as many times as you need, for as many projects as you have to list.
Please fill this form out in its entirety. If you do not complete the form fully it will not let you submit it.
