This form is to confirm or deny permission for a middle school student to read a high school level (young adult) book. These young adult books contain content that may include advanced reading levels, mature subjects/topics, graphic violence, and/or profanity.

I understand my child is requesting to a read a book or books with content at a higher maturity level and/or reading level and approve of his/her request.

I do not give permission for my middle school child to read a book or books that are reserved for high school students.
