As parent/legal guardian of the child listed above, I, the undersigned, hereby give permission for that child to participate in any Adult Co-Ed Soccer League activities to take place at the Wright National Soccer Campus.

The League is administered by the Central New York Soccer, LLC with fields provided by The Oneonta Soccer Club. In the event of an emergency requiring medical attention, I hereby grant permission to a physician or other medical, paramedical, or hospital personnel designated by Adult Co-Ed Soccer League Staff to attend my child.

I hereby release Central New York Soccer, LLC, the Oneonta Soccer Club, The Otsego County Industrial Development Agency, any league sponsors, soccer league organizers, or league affiliates and volunteers from any liability for any injury incurred by my child while engaged in any adult soccer activity. I understand that participating in this athletic activity is potentially dangerous and that physical injury or death may occur to my child as a result of his/her actions or the actions of other participants in the program.

I also understand there is a risk of transfer of communicable diseases to my child through participation in the league such as, but not limited to, Influenza, COVID-19, tuberculosis, whooping cough, etc. and that the State of New York has categorized soccer as a mid-level risk sport due to limited physical contact.

I agree to review, prior to my child’s participation, the conditions of the playing fields and related facilities and equipment used in these activities and will accept any risks they might present while he/she is participating. I also understand that shin guards are mandatory. I hereby release Central New York Soccer, LLC, the Oneonta Soccer Club, The Otsego County Industrial Development Agency, any league sponsors, soccer league organizers, or league affiliates and volunteers from any liability for any injury incurred by my child or sickness he/she may contract while participating in or traveling to and from the fields in order to participate in this activity.
