Kevin Proch Memorial Scholarship Application

To honor Kevin Proch and his contributions to NTHS athletics and the North Tonawanda Recreation Department, his family and friends are establishing the Kevin Proch Scholarship Fund. 2 seniors who were high school athletes AND a past participants of a Recreation Department program, will receive a yet to be determined amount to be used towards post high school expenses.

KEVIN PROCH MEMORIAL SCHOLARSHIP:

1. Awarding two (2), yet to be determined amount, scholarships
2. Participated in NTHS Sports Program(s) for your freshman, sophomore, junior, OR senior years.
a. List all teams and positions played
b. Provide the names of NT head coaches you played for
3. Participated in a North Tonawanda Recreation Department Program(s)
a. List programs and years participated in (general idea if there are many)
4. Submit a (REQUIRED) typed essay of not more than 500 words: If you are submitting your essay separate from your application, please double space.
a. Share what impact NT athletics and NT Recreation have had in your life.
5.. Outline your need for financial assistance. Include 1 goal you'd like to accomplish in the first 5 years after high school.

List any sports played during your tenure in high school. Include modified, junior varsity and varsity experience. List all teams, years played and positions played. 
Provide the names of the head coaches you played for and the corresponding sport.
High School Sports
Please note, applicants are not required to have played a varsity sport to be eligible for the Kevin Proch Memorial Scholarship. Any level of play in high school will count.
List North Tonawanda Recreation Department programs you participated in, including the years you participated.  (General idea if there  are many)
In an essay of no mare than 500 words, please tell us what impact participating in an NT high school sanctioned sport, AND an NT Recreation Department program, has had on your life.
Outline your financial needs after high school and tell us about just 1 goal you'd like to accomplish in the first 5 years after graduation.
