TA Excellence Award (2020-2021): Student Nomination Form
APPLICATION DEADLINE: December 17th, 2021

Any supporting materials (optional) must be emailed to Katherine Duncan at katherine.duncan@utoronto.ca by the deadline. Please make sure you indicate in your email the name of the teaching assistant being nominated. Please see the Award Description for further details.

This form may be submitted by any student or group of students from a PSY course with an eligible TA. In the case of a group nomination, please select one person as the contact person. If applicable, this should be the person who will be submitting any additional supporting documents.
