High Schools and their students should use this form to request a semester-long learning experience at Parkview. The information you submit will allow Parkview Health Education Services to match high school students with the best learning experience we can offer through our system of healthcare opportunities.
Submitting a request does not guarantee your placement in one of our learning opportunities.
Schools and Students should not contact Parkview departments directly. All placements for learning experiences must be made through Education Services.
Process for a high school semester-long placement at Parkview:
- All requests must be submitted by the school.
- All requests must be received no later than March 1 for consideration for the next school year. If you are making a request after March 1, please contact one of our high school consultants for consideration: Allen County Schools - Rachel.Wild@parkview.com or Non-Allen County Schools - LaShay.Montana@parkview.com.
- Parkview's Education Services Consultant will work on finding a placement for the student(s) as there are placements available in the student's desired locations.
- School/Student will receive confirmation or denial of placement by May 1.
- If confirmed, student(s) must return the Attestation Form and the Accepted Student form by July 1st. Failure to provide both completed forms will prevent the student from beginning at requested start date or lose the placement.
