Diploma Replacement Request

This form will be used to process your replacement diploma. The cost of a replacement diploma is $100 per diploma which must be paid prior to ordering your diploma from the vendor. The Bursar's Office will contact you for payment. Upon verification that the fee is paid, your replacement diploma will be ordered. Replacement diplomas will be shipped directly to you from the vendor.
