2021 Eugene-Springfield Youth Orchestras Summer Instrument Rental Agreement
Thank you for giving your student the opportunity to continue to improve and play! Please fill in the following form to continue renting your ESYO instrument over the summer.

The summer rental period is June 2021 - Sept 2021 and the fee is $40.00.
Once the form is submitted the office will email you an invoice and payment will be due upon receipt.
