Impact Assessment of ''Table Top'' Training Program
Dear respected respondent,


This is a pilot version for the study on Impact Assessment of MyOHUN One Health Training Program conducted by the National Coordinating Office (NCO) of MyOHUN. This research team is led by Assoc. Prof. Dr. Roziah Mohd Rasdi from the Department of Professional Development and Continuing Study, Faculty of Education Universiti Putra Malaysia.  

This survey aims to examine the impact of MyOHUN training programs among the training participants. Specifically, it involves Table Top training program in which you have participated over the last 5 years. This survey is divided into three sections, i.e., Section A, Section B and Section C.

Section A includes questions related to your Personal and Professional Background

Section B comprises items on the training impact based on three levels of evaluation, namely, (i) Reaction, (ii) Learning and (iii) Behavioral Change.

Section C includes questions on Behavioral Change for academicians who have been involved in the training program.

Please be informed that there is no right or wrong answer, and all responses will be reported in aggregated form.

Thus, rest assured that the respondent’s identity shall remain anonymous.

Furthermore, data collected from this study will be treated as confidential and will be used for academic purposes only.
