Terms and Conditions:

 - There will be no refund or make up sessions if the member wishes to discontinue the program after the commencement or forego a training appointment without 24 hours advanced notice.

- Failing which, the session will be deemed as a no show and will be charged in full.

 - Members may cancel sessions without charge by giving 24 hours prior notice.

 - The Club is not responsible for any injuries or disabilities during training sessions.

 - I have read and understood the above terms and conditions governing my participation in the above program. I abide by the above conditions and further agree that the Hollandse Club and the Contractor will not be liable for any injury or loss suffered because of my participation in the program.

 - I shall abide by the above statement by performing body checkups on an annual basis with my doctor.
