This form is still a work in progress, but I have published it to get your orders started during the COVID-19 lock out. Our awesome Davines distributor has offered to ship directly to our clients! I will place all these orders manually with them once submitted.

After you fill out this order form, I will email you an invoice, that will need to be paid through the invoice by bank payment or by Venmo @modernlovesalon. We will contact you if we need more information. All orders are shipped upon receipt of payment.
