Payment Details-Please provide as much detail as possible. If paying with a Purchase Order please either attach that here or let us know the method in which the PO will be sent (email, snail mail, etc.) . If you are paying with a school check and need an invoice sent to the school please indicate the contact information for the invoice. If paying with a personal check please note if you need a receipt of payment prior to the workshop date.
