If you are attending a Berkeley Schools summer school program and plan to use the Chromebook you were issued for Distance Learning over the summer please fill out the requested information in this survey to be sure your Chromebook is not be automatically disabled.

Asset Tag Number *
The asset tag number is located on the sticker on the sticker on the underside of your Chromebook. It is the 4-6 digit number below the barcode. Please enter the Asset Tag number in this field.

Have you been issued a hotspot?
If you no longer need the hotspot please return it to your school so it can be issued to another student and select No/Returned. Service on hotspots not registered will be automatically terminated at the end of June.
