Women's Health Survey
Community Living London is working with the Community Engaged Learning Program from the University of Western Ontario to develop educational resources for the people we support about women’s reproductive health throughout their life time.  This resource will include topics such as menopause, perimenopause, birth control, menstrual health etc.  The goal of this project is to create resources to assist women to learn and understand the cycle of their reproductive health.

In order to develop this resource, we are gathering information from women supported, their families and care providers.  We appreciate you taking the time to complete this quick survey.

Please note participation in completion of this survey is voluntary.

All information gathered from this survey will be used by Community Living London only and not include any personal identifying information, nor will any of the findings will be published.
