Medical Assisting Disclosure
Welcome to Medical Assisting! Congratulations on starting early to make your goals a reality. I am looking forward to working with each of you.

IMPORTANT NOTE 1: The Medical Assisting class recommends previous or concurrent completion of Medical Anatomy and Physiology. There is a state test at the end of the year in this subject for the Utah Endorsed Medical Assistant Certificate. We will review each body system 4th quarter, but not be able to fully teach this subject, as there is not sufficient time with the MA curriculum.

CLASSROOM RULES
1. Be in assigned seat, ready to work at start time. Stay until class ends.
2. Clean, appropriate language.
3. No missing assignments.
4. Electronic devices used WITH PERMISSION ONLY.
5. Everyone is treated with kindness and respect.
6. Clean up after yourself.

IMMUNIZATION REQUIREMENTS
Proof of immunizations is required to participate in externships.
The following are needed, and may be obtained from your family physician to County Health Department.
 5 total Tdap injections - the last one being within the last 10 years.
 2 MMR injections
 2 Chickenpox (Varicella) injections, or titer
 2 complete TB tests at least 1 week apart or Quantiferon titer.
 1 flu shot dated after September 1.
 3 Hepatitis B injections, and Hep B titer. If Hep B titer comes back low or non-reactive, subsequent and documented action is required.

CERTIFICATIONS OFFERED
The Utah State Board of Education requires 4 Skill Exams that, when averaged together, must have a score of 80% or better in order to receive the Utah Endorsed Medical Assisting Certificate. These skill tests are:
1. Medical Terminology for Medical Assistants (taken at the end of the first semester)
2. Clinical Laboratory
3. Medical Office Management
4. Anatomy and Physiology
Earning your Utah Endorsed Medical Assistant Certificate qualifies you to take the National Certification Exam, which is optional, but beneficial for employment. The National Exam must be passed with 70% or better to earn the National Certificate.

GRADING
1ST, 2ND, 3rd Quarter: 35% Assignments - 35% Tests - 30% Professionalism
4th Quarter: 50% externship evaluations - 20% Assignments - 20% Tests – 10% Professionalism

ACADEMIC PERFORMANCE REQUIREMENTS
1. Must maintain 80% or better to qualify for an externship. (Utah State Board of Education Requirement) Students not meeting this requirement will be asked to drop the class at the beginning of 2nd semester.
2. Assignments: 10% deduction if not turned in prior to class starting.  20% for 1 day late, 2 or more days late,  50% is the highest possible score.
3. If a student is absent the day an assignment is due, it MUST be turned in the day the student returns to class to receive full credit. Otherwise the above point deductions apply.  Longer absences need to be discussed with the teacher for appropriate due dates.
4. Tests and Assignments:  All tests and assignments must be passed with 80% or better. If a score falls below this range, students are allowed to retake/redo tests/assignments until 80% or higher is reached. The test scores will be averaged together for the student’s final score.  
5. If a student is absent on the day of a test, it must be made up within 1 week of the original test date to receive full credit.
6. These requirements also apply when students are out for externships. Assignments must be turned in on the day class meets, or students will need to make a special trip to the classroom to turn in the assigned work.
7. If the student’s academic performance falls below 80% when they are out for their externship, they will be required to put the externship on hold, and return to class until their grade is in the required 80% range.

PROFESSIONALISM REQUIREMENTS
1. Lack of professionalism is the #1 complaint of office managers for new employees, with cell phones being the biggest annoyance. Grading for professionalism will be based on classroom behavior and be conducted as follows:
a. Students are given 20 professionalism point per day (tallied at the end of each week). Points are deducted for class rule infringements or any other behavior that would be inappropriate in the classroom or workplace setting.
b. Deductions are usually 10-20 points, but it is up to the discretion of the teacher. Repeated violations will result in greater point deductions and conferencing with parents and/or principal.
c. Sluffs and cheating will result in all points for the week being deducted, and a meeting with the principal and parents will be necessary before a student can return to class.
d. If professionalism becomes a chronic problem, the student may be asked to leave the class at the end of the first semester.
e. Any cell phone complaints from an externship clinic will result in an automatic step down grade deduction.
Attendance Requirements: Class attendance is vital to your success!! College MA courses are up to 18 months long. Our course must be completed in 2 semesters, which means we cover important material every day! In order to stay caught up and current you need to be here. Any more than 3 absences and/or tardies will result in an automatic step down grade deduction.

IMPORTANT NOTE 2: Our textbooks contain anatomically correct drawings and photos. This includes the reproductive system. Care should be taken that textbooks not be taken out in other classrooms. If you or your student are uncomfortable handling this aspect of the curriculum, please reconsider taking the Medical Assisting Course.

LABORATORY
1. Students are required to perform the following laboratory tests and clinical procedures on each other:
    a. Positioning                                                                          e. Urinalysis
    b. Draping                                                                                f. Throat cultures
    c. Vital signs                                                                          *g. Injections
    d. Capillary finger sticks (for blood analysis)                      h. Phlebotomy

EKGs will also be performed, but adult male volunteers are brought in for teaching this skill.
*Injections include arm, stomach, thigh and hip. Private body areas are covered at all times.

2. If you do not want your student to participate in these procedures, please withdraw from the class. Please understand it is unreasonable for a student to perform these procedures on a classmate, and then refuse to be a “patient” for someone else. Also, being the care-giver and patient teaches empathy and understanding.
3. We will be handling body fluids in our lab, and will follow all Standard and OSHA safety procedures. Any student not taking lab procedures seriously will have lab privileges revoked, and no credit given.

IMPORTANT NOTE 3: Students are NOT ALLOWED to give any kind of injection or practice phlebotomy without the direct supervision of a licensed Jordan School District nurse. Students violating this rule will be asked to drop the class.

DRUG AND BACKGROUND TESTING
1. Drug testing is required by the Utah State Board of Education. This test must be done on a random basis, so students will not know the testing date in advance. If the student does not pass or refuses to take the drug test, they will be asked to drop the class.
2. Background checks are also required.  This needs to be completed by the student and parent at BCI, then BCI sends the information to us.

EXTERNSHIPS
1. It is the responsibility of each student to find a clinic to complete their externship. A Job Seeking Skills unit (including resumes, interview skills, etc,) will be taught in preparation for this. It is also the responsibility of the student and parent/guardian to provide transportation to and from the externship site.
2. Students may be required to help patients disrobe and/or assist in performing procedures that expose patients’ private body areas. By signing this disclosure I am giving consent for my student to perform these duties.
3. Externship site should be secured by February 1, 2020 in order to give us adequate time to complete the necessary paperwork.

ONLINE SCHOOL - WHAT WE LEARNED FROM COVID
Should it be necessary to go to online school again, please understand the curriculum and grading requirements don't change.  Last year when students went to online, many of them picked up extra work hours and found it very difficult to keep up with classwork.  Please keep in mind that school will still be in session - just conducted differently.

I have read and understand the requirements and policies as outlined in this disclosure statement. I understand and give consent for my student to perform the laboratory skills outlined above. I also give consent for these laboratory skills to be performed on my student while in the Medical Assisting class. I also give consent for a drug screening and background check to be performed. I understand I cannot continue in the class if I do not follow the rules and regulations of the Medical Assisting Program.
