This questionnaire will help us to identify the benefits and value of the Home Library Service, so we can ensure it is providing the service our readers need. We will periodically re-issue this survey so we can monitor the success of the service and our progress in improving it.
This survey consists of multiple choice questions with options to write more, and it has a space for you to write a longer message at the end. It shouldn't take more than ten to fifteen minutes to complete.
Every volunteer-reader relationship is different, so some of these questions may not apply to you. If that is the case you can just select 'other' and enter N/A.
It's really important you understand that this survey is not assessing you in any way; our aim is to identify and evaluate the impact the service has on readers. To reassure you on this, the survey information is collected anonymously, so neither you nor your reader will be identifiable. Please be honest so we can make sure the service is as effective as possible.
We are aware you cannot know how your reader feels and thinks but please answer as best you can, based on your experience. You are welcome to work through the survey with your reader if you wish, but we are not expecting you to.
If you are a volunteer with multiple readers, we ask you to fill in an individual survey for each; this allows us to get an accurate measure.
