Living Water Community: Ministry for Migrants and Refugees estará asistiendo a dos jornadas de vacunación para los niños entre 12 a 18 años de edad. Las jornadas se llevarán a cabo de la siguiente manera:
- Sábado 21 de Agosto de 2021 en la Academia Nacional de Artes Escénicas (por sus siglas en ingles NAPA), Port of Spain (cerca de la oficina del Living Water Community)
*Por favor tome en cuenta que la inscripción para el sitio en NAPA se llevara a cabo en la oficina del Living Water Community ubicada en la esquina de Gordon y Frederick St. Luego las personas serán dirigidas al NAPA para sus vacunaciones- Sábado 28 de Agosto de 2021 en Divali Nagar Site, Chaguanas (cerca de Price Smart)

Las vacunas Pfizer BioNTech COVID-19 serán administradas en ambos días, y son seguras para los niños de 12 años en adelante. Estarán presentes los intérpretes.  

Se les pide a los Padres y Representantes apartar una cita a través de este formulario. Se prefieren las citas, pero aquellos que asistan sin cita previa serán acomodados entre las 11 am y 3 pm. Si cuida a más de un niño/a, por favor aparte una cita para cada niño/a que necesite ser vacunado.

Living Water Community: Ministry for Migrants and Refugees will be supporting two COVID-19 vaccination drives for children 12-18 years old. The drives will take place as follows:
- Saturday 21st August 2021 at the National Academy for the Performing Arts (NAPA), Port of Spain (near to Living Water Community office)
* Please note, registration for the NAPA site will take place at Living Water Community office on the corner of Gordon and Frederick St. Persons will then be directed to NAPA for their vaccines.
- Saturday 28th August 2021 at Divali Nagar Site, Chaguanas (near to Price Smart)

Pfizer BioNTech COVID-19 vaccines will be administered on both days, and are safe for children 12 years old and above. Interpreters will be present

Parents and Guardians are asked to book an appointment through this form. Appointments are preferred but walk ins will be accommodated between 11:00AM and 3:00PM. If you care for more than one child, please book a separate appointment for each child that needs to be vaccinated.
