I understand that she must abide by the rules and regulations set forth by the Coach, Administration, and Knox County Schools. I understand she is required to attend ALL OF THE FOLLOWING: camp, junior camp, practices, football and basketball games, Homecoming and other fundraisers, competitions, and community events. I understand violation of any of these rules may lead to temporary or permanent suspension from the squad. (Parent- Type Full Name) *
I understand that tryouts will be completed in accordance with Knox County policy and we agree to abide by these decisions.                                                             I understand the financial and time commitment associated with Farragut High School Cheerleading.           I understand by the very nature of the activity, cheerleading and tumbling carry a risk of physical injury. No matter how careful the participant and coaches are, how many spotters are used, or what landing surface is used, the risk cannot be eliminated. The risk of injury includes muscle pulls, dislocation, broken bones, stitches, and concussions. The risk also includes catastrophic injuries. I understand these risks and will not hold Farragut High School or any of its personnel responsible in the case of accident or injury at any time. (Parent- Type Full Name) *
