May 26th Parental Request for Student Absence
SM West 9th-11th graders will be taking their finals May 24-27th.  Attendance is very important for their exams.

May 25th has a built in seminar that can be used for review time.
May 26th has built in time for Make up testing.  
Every student may not need additional time for these blocks.

Parents/Guardians may use this form to request that their student be EXCUSED from:
May 25th Seminar - 1:10pm-2:40pm                        
May 26th Block 3 - 10:50am- 1:05pm
May 26th Block 4 - 1:10pm-2:40pm
 
Please be aware that all students that are excused from these blocks must leave SM West campus and will not be able to use bus transportation at the end of the day.  Students will be able to eat a school lunch from 10:50-11:20, but then must either walk home, or obtain a ride home.
