COVID-19 Release of Liability - VOLUNTEERS ONLY

PLEASE READ BEFORE SIGNING:
In consideration of being allowed to participate in any sports programs at Valley Athletic Association (this includes all practices and games), the undersigned acknowledges and agrees that:

1. Even throughout the changing of the colored “phases” within the state of Pennsylvania, there are still risks of contracting the COVID-19, and any other illnesses.
2. Within those risks of contracting also includes the risk of injury, potential for short-term and long-term effects up to, and including death.
3. Abiding the CDC, local, and state guidelines provides no guarantee of safety.
4. While participating in team sports, there is a risk of contracting COVID-19, medical conditions, diseases, illnesses, or any other potential pandemic viruses.
5. I willingly comply with the stated and customary terms and conditions set forth by Valley Athletic Association, Bensalem Township, and the State of Pennsylvania.
6. I, for myself, and on behalf of my heirs, assigns, personal representative, and next of kin, hereby release and hold harmless, without liability, the Valley Athletic Association, Bensalem Township, State of Pennsylvania, and their officers, officials, agents and/or employees, other participants, sponsoring agencies, directors, sponsors, advertisers, volunteers, and any owners or lessors of the premises used to conduct the event with respect to any and all injuries, illness, disability, death, or loss or damage to a person or property incident to my child’s involvement or participation in these programs, whether arising from the negligence of the releasees or otherwise, to the fullest extent permitted by law.
7. I, for myself, my spouse, my child, and on behalf of my/our heirs, assigns, personal representatives, and next of kin, hereby indemnify and hold harmless all the above releasees from any and all liability.
8. I, the parent or guardian, assert that I have explained to my child the risks of the activity, their responsibilities for adhering to the rules and to this agreement, and that the child understands the agreement.
By signing this document, I sign for myself, my spouse, other household members, spectators, and child/ward, and have read this release of liability and assumption of risk agreement, fully understand its terms, understand that that we have given up substantial rights by signing it, and sign it freely and voluntarily without any inducement.
9. Failure to comply with rules and regulations set forth by Valley AA, Bensalem Township, or the State of Pennsylvania can and will result in removal from grounds and termination of registration contract - without refund
