Parental consent is necessary for Gonzales Preschool staff to take appropriate action on behalf of a child. If a child is ill or requires medical attention, the parent/guardian will be notified. If Gonzales Preschool is unable to contact the parent/guardian in the event of an emergency, written consent is required for Preschool staff to call for help. This form will accompany the child to the emergency service.
