Please fill out this survey to let us know if you would or would not like your child and other family members to be vaccinated by trained medical personnel at a vaccination clinic organized by the Department of Health and held here at Bridge for our students. We would also schedule a clinic here at Bridge for your family's second dose.

All vaccinations are FREE.

All students under 18 would need to be accompanied by an adult to be vaccinated.

When making your decision, please note that school will be IN-PERSON for summer school and next year; remote learning will NOT be an option for the 2021-2022 school year.

Vaccination is the best, easiest way to keep you and your loved ones safe from Covid 19. We STRONGLY encourage all eligible people to get vaccinated.
