Online Music Ed classes
We are now including Music Ed (Music Theory and Sight Reading weekly classes) in the sign up. We made these into group classes now so we can afford to include the full production music video that the students requested. If you want private Music Ed classes you still have that option but will need to pay for the private instruction (but at a 50% discount). Seeing as almost all students in Varstiy are on the last level required before you can opt out of Music Ed (Level 3), we figured group classes would be more efficient.

Please select a minimum of two options on the times listed below (you may select as many as you are available for). These do not get make-ups but you are able to drop in on any other class in the week if you are unable to make the one you are scheduled for providing. Further more, if you feel that you are going to have a busy second half of your semester with finals, you can take multiple classes in the week until you have take your required amount of classes (15 hours - 30 x 30 min classes)  Daily sight reading is actually INCREDIBLY beneficial for progressing faster so we highly recommend this if your schedule allows.

For those that opt for private classes these are discounted at 50%  and you will be entitled to 2 make-ups per semester.  (Normally priced at $85/hour we are offering them at $40 just for Varsity Students)

All Varsity students are required to take Music Ed until Level 3 is fully completed.  This includes both Music Theory and Sight-reading.

It was requested that we make the 'at home' box music videos optional this semester. These will include full costumes.  Those who do not select the extra music videos will not be eligible for solos on the album for these songs but you may still audition for them for live performances.

I understand that these purchases are non refundable and agree to have the card I have on file with LJMA / Squad Harmonix charged for the final amount once I receive a final invoice after this order is placed. Invoices will take 1-2 days to be completed once received  (if we don't have a card on file yet we will call you this week for payment) Payment must be received before the start of the first class.  I understand that make ups for Squad Music Theory classes are not on available as the classes are offered at a discount available only to current Squad Members.. *

MEDIA CONSENT FORM AND RELEASE FOR MINOR CHILDREN
I am the parent/guardian of the above child I am registering in this form (“My Child”). I hereby grant Squad Harmonix LLC (“Squad Harmonix”) and its agents, successors, and assigns the absolute right and permission to use My Child’s name, likeness, image, and voice in photographic portraits, pictures, digital images, audio recordings, digital recordings, and/or videotapes of My Child, or in which My Child may be included in whole or part, or reproductions thereof in color or otherwise, in any and all media, including the internet, for any lawful purpose, including but not limited to use in any Squad Harmonix publication, on any Squad Harmonix websites, streaming platforms, albums, E.P. and/or single releases, and/or on any social media page or account of Squad Harmonix, including, without limitation, the Facebook and Instagram pages or stories of Squad Harmonix and its members, without payment or any other consideration. This consent includes the unrestricted right and permission to copyright and use, reuse, publish, republish, edit, alter, exhibit, and/or distribute images and audio recordings of My Child or in which My Child may be included.

I hereby waive any right that I may have to inspect and/or approve the finished product or the copy that may be used in connection therewith, wherein My Child’s likeness appears, or the use to which it may be applied.

I hereby release, discharge, and agree to indemnify and hold harmless Squad Harmonix and its agents from all claims, demands, and causes of action that I or My Child have or may have by reason of this authorization or use of My Child’s photographic portraits, pictures, digital images, audio recordings, or videotapes, including any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form, whether intentional or otherwise, that may occur or be produced in the taking of said images, audio recordings, or videotapes, or in processing tending towards the completion of the finished product, including publication on the internet, in brochures, or any other advertisements or promotional materials. I understand that I have the right to revoke this consent at any time, in writing, but that any such revocation shall not affect disclosures previously made prior to its receipt. I represent that I am at least eighteen (18) years of age and am fully competent to sign this Release.

I agree never to share, send or copy any 'Squad Harmonix Practice Materials'. This includes customized sheet music, practice tracks / recordings and any music theory / sight reading and course materials and documents.

Participant Code Of Conduct for Online Classes
Private Classes:

Participants will arrive early to every scheduled class and be ready to participate as soon as class starts.
Participants agree to come to class prepared with the correct equipment required for each class. This includes but not limited to:  
Charged devices capable of accessing Zoom with the ability to turn on ‘original sound’
Sheet music printed (if required)
Pencils, highlighter and various colored pencils (if desired) to mark sheet music.
A second device separate from device with Zoom that has the LJMA Student Dropbox folder loaded and ready to playback tracks or access homework.
Sharing Zoom meeting links and passwords is strictly prohibited.
Personally recording any LJMA session is prohibited without permission.
Only registered participants and families are permitted to watch/participate in the sessions.
Monitor any background activity in the participant’s camera viewing field that may become visible to the teacher and other students.
Participant user names should not be inappropriate, offensive, or vulgar in nature and must include clearly identifiable information about the registered participant, such as first name or recognizable nickname.
Background images may be used must not be distracting to the teacher or lesson.  Teachers reserve the right to disable a participant’s video and sound if deemed necessary.

Group classes

Zoom Chat: Texts unrelated to the lesson are distracting to both the teacher and class, therefore use the Zoom chat feature only for texts relating to the lesson. Teachers reserve the right to disable all chat functionality if deemed necessary.


All participants must abide by the LJMA Participant Code of Conduct as outline above.  Should a participant not be in compliance with the Participant Code of Conduct, LJMA staff should report the issues to the parent/guardian and the appropriate facility if applicable. Failure to follow these rules may result in denial of program participation privileges.
