Please submit this form to inform the school  if your child will be absent / late or require early dismissal for a particular day or period of time.

Please notify the school using this standard form by 8:00am of each day to ensure our timeous capturing of your request.
If you intend for your child to be absent due to holiday or other reasons during the semester OR if you intend for your child to arrive late or leave early on any particular day - use this form to indicate the reason.
 
The school has an automated system to alert parents of student absences as an additional level of security. With your support in consistently using this form, we are aiming to reduce the notification delay for unauthorised absence which keeps your children safer.
