1)  Complete the form below to indicate which items you would like to purchase.  If you want multiple quantities of the same item, you will need to make a note of that in the Additional Comments box at the end (i.e. 2 small youth ohio shirts).
2) There are limited quantities so you will receive an email within a few days to let you know the total amount, who to send the check to, and if any items are out of stock.    Check or cash payments only.  Please make checks out to SPB PTO.
3) Your order will be delivered to SPB by Christmas Break.
