Kindly fill out this online request form for the issuance of your Cash Loan Statement of Accounts (SOA). Your SOA contains all relevant information about your loan balance, as well as the detailed payment guidelines for the settlement of your account.

SAIS ID *
Format: 10012345; If no SAIS ID type 10000000
Desired Amount to Settle *
If partial payment, enter your desired amount on the "Other" textbox. Note that your hold in SAIS will be removed after you have paid the full amount of your loan.  
Desired Date of Payment *
Please be informed that all SOA requests submitted after 5:00PM on December 21, 2023, will be processed/issued on January 2, 2024. Thank you.
