EPN Trivia Night Team Sign Up Form
On Saturday, October 24 from 7-8 pm, Rotaract District 5160 is hosting a Trivia Night in honor of World Polio Day. All proceeds from this event will be donated directly to Rotary International's End Polio Now initiative.

Form a team of 3-5 of your friends and tune into our Twitch at 7 pm! We will be broadcasting the questions there. The Twitch url will be emailed to you prior to the event. Make sure you have set up a way for your team to communicate, such as Zoom or Discord.

The team that answers the most questions and the team with the best name will receive a shoutout!

The fee to participate is $5 per team, which you can pay to our Venmo, @Rotaract5160. Please put your team name in the payment description so that we can keep track of who paid.
